Operator Handover Template

Use this template at the start of every agentic deployment. Fill it in before the agent runs in production, not after.

Sections

• Trigger conditions — list every condition under which the agent must stop and ask for a human.
• Owner — name a single accountable operator per shift.
• Escalation route — primary and secondary, with response-time targets.
• Audit fields — what the agent must log on every handover.
• Reversal procedure — how a decision is undone, and within what window.
